NAME: The name of the Forum shall be GUJARAT STATE FAMILY PHYSICIANS` FORUM (GSFPF).
AIMS AND OBJECTIVES:
(1) To promote and advance Family Medicine amongst the
members & Community.
(2) To maintain the honour and dignity and to uphold
the interest of the medical profession and to promote the co-operation amongst
the members.
(3) To achieve equality among all members of the
profession & other professional associations like I.M.A. & FFPAI
OFFICE: The office of the GSFPF shall be at
headquarters – i.e. at Ahmedabad
ELIGIBILITY
OF MEMBERSHIP:
The members who are eligible to become member of
I.M.A. can become member of GSFPF.
CLASSIFICATION
OF MEMBERS:
1. Institutional Members: I.M.A. eligible member of any FPA can become institutional members.
2. Individual Members: Any individual having eligibility of membership & his Head Quarter
is not having institutional membership can join as individual member.
3. Founder Members: All the members whether Institutional or Individual enrolled in the
register before 31st.March 2008 shall be Founder Members.
4. NRI
Members: The eligible members residing outside India and enrolled in the register
shall be known as NRI Members
FORUM
YEAR:
Financial Year and Administrative year of the
Forum is: 1st April to 31st March.
YEARLY SUBSCRIPTION of FEES:

Individual Members: Rs. 50 per year before 30th April

Membership:
For
all units and individual members 20 times of annual fee.
(These
fees are subject to revision time to time; the unit has to pay difference of
fees applicable at that time when membership strength changes.)

ZONAL COMPARTMENTS
(a) Headquarter Zone: Ahmedabad District& Gandhinagar District.
(b) West Zone: Comprising of all districts of
Saurashtra and Union Territory of Diu
(c) North Zone:
Comprising of all districts of North Gujarat
i.e. Mehsana, Sabarkantha, Banaskantha.
(d) Central Zone: Comprising of all districts of
Central Zone i.e. Vadodara, Anand, Kheda, Panchmahal, Dahod.
(e) South Zone: Comprising of all districts of
South Gujarat i.e. Surat, Bharuch, Navsari,
Narmada, Valsad, Vapi, Dang and Union Territories of Daman
and Dadra Nagar Haveli.
(f) Kutch:
Kutch District
(4) OFFICE BEARERS:
President:
The president shall be from each zone by rotation.
Vice-Presidents:
Six in number. One from each zone. By rotation in alphabetical order amongst
the local branches.
Honorary Secretary:
From HQ Zone.
Honorary Joint Secretaries: Two in number. One from HQ Zone & one from President’s Zone.
Treasurer:
From HQ Zone.
